Copy of SD 600 (DoDDS Student Registration Form)

  1. Student(s) must be present with sponsor for a bus pass photograph.
  2. Address verification:
  • New registrations or recently moved
  • On base:  A housing assignment letter showing building number
  • Off base:  A copy of lease (or any bill showing address) and map
